Camp Calvin Forms
Registration Form
Complete the Registration Form, and mail with a $50.00 registration fee payable to Camp Calvin. The total camp fee is $300.00 (includes a camp T-shirt) if registered before May 1. If the registration is received after May 1, 2010, the total camp fee per camper will be $310.00 (includes a camp T-shirt). All balances are due May 20.

Health Form
Give completed Health Form to your church's contact person (or Steve Potter) no later then May 20. Form must be printed on a single sheet of paper (duplex). Be sure the health form is signed by a parent or guardian. A camper must have a completed health form in order to attend camp.
Cancellation Policy
Cancellations for any reason that are received before May 20 will receive a full refund. Cancellations received after May 20 and until the beginning of camp will not receive a full refund except for medical or other emergency reasons (death in the family, etc.). No refunds will be made for dismissals due to disciplinary action, late arrivals, or early departures.
